What is a Senior Care Pharmacist?

A pharmacist that specializes in the use of medications for management of acute or chronic conditions in older adults. A senior care pharmacist closes the gap of unmet clinical needs and is a necessary component of any medical team that is focused on improving patient outcomes.

The Premier Pharmacist:

  • Utilizes knowledge of geriatric medication principles to elevate the quality of care provided to our patients by collaborating with our team of clinicians towards the common foal of ensuring all medication regimens are safe, effective, and remain clinically appropriate

  • Delivers education to our providers and facility staff on a variety of medication related clinical and regularity topics. Premier is particularly focused on increasing awareness of the importance of medication optimization and the need to deprescribe unnecessary medications and our education activities reflect this core ambition.

  • Provides a pharmacist’s perspective and clinical insight to have a substantial impact on reducing adverse drug effects, reducing medication related ER visits and hospital stays, and most importantly, gives our patients an overall enhanced quality of life.
