Holly Roy, PA-C
VP of Clinical Operations
Holly Roy, PA-C completed her undergraduate studies at Bridgewater College and completed her Master's Degree at Jefferson College of Health Sciences in 2011. Her residency and course work contributed to a vast experience in family practice, emergency medicine, rural health, and leadership. Holly practiced in family medicine and urgent care prior to finding her passion with post acute care. She started with Mid-Atlantic ElderCare which was founded by Dr. Goad, the CEO of Premier. Holly quickly gained knowledge of the space and was a natural with predicting operational needs of the organization.
Holly started her current position of COO in 2021, and in a very short time, has been instructional in the the expansive growth of Premier. She has assisted not only covering clinical needs of patients, but with business development, planning and implementing new company ancillary services, contract negotiations/renegotiations, staffing needs, as well as having a pivotal role in a large practice acquisition. She is a contributing author to Geriatric Notes which was authored by our Chief Education Officer, Jamie Smith. Holly also serves her community as a Local Medical Examiner in Bedford and Campbell Counties.
In her spare time, Holly enjoys weight lifting, spending time outdoors, working on her family farm where she and her family raise Black Angus beef cattle. She resides in Bedford, VA with her husband and three young children. -
Kim Berchen
VP of Development and Value-Based Care
Kim is a healthcare executive with over 25 years of operational, growth, and leadership experience within the healthcare delivery system. Fifteen of those years specifically in the post-acute space with primary care and behavioral health.
Throughout her career, Kim worked with multiple organizations, from start-ups to large corporations as a VPO and as a consultant improving operational efficiencies and leading growth. She has also integrated newly acquired practices into new, established cultures.
In her former role as Vice President of Ops for other organizations, she developed, implemented, and operationalized two new service lines including behavioral health and Nocturnist Practices. She also oversaw the operational and financial performance for over 350 clinicians across the country. As a former Clinical Practice Manager, she was charged with managing and growing post-acute practices that increased providers by 200% with a 95% retention rate.
Kim has a BS in Psychology with emphasis on Organizational Behavior from Rollins College.
Originally from Texas, Kim has lived in the mountains of North Carolina and now Tennessee for the past 10 years. She enjoys hiking, reading, traveling, and exploring new places with her family, friends and Charley, her golden retriever.

Jamie Smith, FNP
Clinical Training Specialist
Jamie Smith is a certified Family Nurse Practitioner (FNP) who provides direct patient care to residents in Roanoke, Virginia, and serves as Premier Geriatric Solutions' Chief Education Officer. She completed her undergraduate education at Liberty University in 2007 and, in 2013, graduated from James Madison University with her FNP. Her vast nursing experience includes medical-surgical, neurology and surgical intensive care, long-term acute care, and emergency medicine. She and Dr. Bradley Goad published a manual for students and new providers who provide care to older adults in the outpatient setting, Geriatric Notes. In addition, she contributed to other publications, published by GAPNA (renal failure), and developed CMEs for other companies (cardiac-related).
Jamie is passionate about improving clinical documentation per CMS guidelines, strives to improve practices through her role and is an educational liaison between PGS and the facilities served. In addition, she has developed and distributed a series of educational tools targeting post-acute-care providers to ensure best practices in the clinical setting.Jamie lives in Callaway, Virginia, with her husband (Kellen Smith) and two kids (Bradley and Madi). When she isn’t working, she enjoys spending time with her family, doing artwork with her daughter Madi, reading, exercising, and playing the banjo.

Barbara Pereira
Vice President of Clinical Pharmacy
Barbara Pereira, RPh is a seasoned Clinical Pharmacist with over 23 years of dedicated experience in the post-acute space. After receiving her pharmacy degree from Massachusetts College of Pharmacy in 1989, her career has spanned from roles as an independent pharmacy owner, a Consultant Pharmacist and Clinical Manager for a large national long-term care pharmacy provider, to most recently holding an executive leadership role where she pioneered the development and implementation of a medication optimization and deprescribing program for a post-acute physicians practice across the country. Her career trajectory has equipped her with a comprehensive understanding of the CMS regulations and survey procedures that govern our industry.
One of Barbara's passions is addressing the complex nature of geriatric medication therapy and reducing the prevalence of polypharmacy in this population. Her mission revolves around educating both providers and clients on potential pitfalls and, more crucially, the available solutions to this widespread industry issue. Barbara is an integral part of the educational initiatives at Premier Geriatric Solutions, collaborating on clinical projects and outcomes-based activities with our leadership team.
Barbara currently resides in Dartmouth, MA with her husband of 33 years, Roy. They have two grown children, Jacob and Rachel, who are out on their own and living their best lives. In her free time, Barbara enjoys hiking or walking at the beach, exploring new dining options, traveling, and particularly looks forward to her annual trips to Portugal to visit with her in-laws.

Heather Allgood
Operations Manager - North
Heather started her medical career as a pharmacy technician, with a long-term care pharmacy which serviced ALF and LTC facilities. She worked as receptionist in the dermatology field, where she created her own position of referral coordinator, working closely with patients and family care practices. Her most enjoyable experience was her 9 years as Medical Supplies Clerk at VMRC. In this position, she was able to have many meaningful one on one experiences with residents that she would meet daily. She collectively has 18 plus years in the medical field, and enjoys being a part of a company that strives to enrich the lives of the geriatric community.
Heather was born and raised in Harrisonburg, VA. She has 5 children (4 grown), and she lives with her husband Fred and their young daughter in Staunton, VA. In her free time, she enjoys spending time with her family at the lake or beach or on an excursion to local thrift stores to hunt for treasures.

Beth Hoke-Windle
Business Manager
Beth has been in the healthcare field for 31 years working as a Certified Nursing Assistant, Payroll Manager, Activities Director, Home Health Aide, Private Duty Aide, Registered Medication Aide, and Director of Medical Records. She brings a wealth of knowledge to the Premier Team and currently serves as our Business Manager. In this role, she goes above and beyond to meet the demanding requirements of our day to day financial operations.
Beth has 3 daughters (Misti, Sierra, and Sommer). She has 3 grandsons (Landyn, Graysyn, and Masyn) and 1 granddaughter (Ava). Beth enjoys spending as much time as she can with her grandchildren.

Tabitha Ellis
Director of Medical Transcription
Tabitha is a licensed physical therapy assistant (LPTA). Her passion for serving others began at the age of 16 when she first became a clinical transporter and receptionist in the imaging department at a hospital. Tabitha later received her PTA degree from Jefferson College of Health Sciences in Roanoke, Virginia (2002). She has over 20 years of post-acute care experience and switched career paths to serve as a scribe for our CEO, Dr. Bradley Goad, in 2021.
Tabitha quickly recognized her passion for serving as a medical scribe and dedicated her time to improving processes for this role. This streamlined approach has served not only the provider but offers timely, complete, and accurate documentation for meeting the residents needs. As the director, she supports the successful recruitment process for hiring and training new scribes. Tabitha enjoys working closely in the onboarding process with new scribes and ensures the transition process is successful for everyone. Tabitha’s commitment to the scribe program continues to gain momentum and interest and has grown exponentially. Several providers who have recently acquired a scribe have commented, “having a scribe has been life changing”.
Tabitha resides in Hardy, VA with her husband, Nick, and their daughter, Raylynn. In her spare time, she enjoys spending time with her family, vacationing in the mountains of Tennessee, scrapbooking with her mom, reading, and working on home projects.
Dr. Bradley Goad, DO, FACP, CMD
Chief Medical Officer
Bradley J Goad, DO, FACP, CMD is a dual Board-Certified Internal Medicine and Hospice/Palliative care physician. He received his BS in Biochemistry from Virginia Tech and his medical degree from the West Virginia School of Osteopathic Medicine. His residency training was completed at East Tennessee State University where he served as Chief Resident at Holston Valley Medical Center.
Dr. Goad has over 20 years of medical experience serving individuals in skilled nursing, long-term care, and assisted-living facilities. He is a Certified Medical Director through AMDA - The Society for Post-Acute and Long-Term Care Medicine He is also co-author, with Jamie Smith, FNP, of a best selling geriatric reference book called Geriatric Notes Jones & Bartlett Learning.
In addition, he proudly served ten years in the Air Force Reserve as a flight surgeon where he was deployed to areas within Afghanistan, Turkey, and Iraq during Operation Enduring Freedom. He was also involved in planning and operational leadership for Medical Readiness Exercises in Nicaragua and Paraguay.
Dr. Goad lives with his wife and son at Smith Mountain Lake. His daughter is currently attending James Madison University. He thoroughly enjoys aviation and is an Instrument Rated Pilot. He also enjoys traveling, reading, barbecuing, and entrepreneurship.
