Premier’s Chronic Care Management (CCM) Program

“Premier’s Chronic Care Management (CCM) program is a progressive and comprehensive tool in our care management tool chest;" reports Holly Roy, PA-C, Chief Operating Officer. With our CCM partner, Holly and her team developed and implemented our current CCM program with the specific needs of the long-term care population in mind. This program has been a critical communication tool, both during and post Covid, we have used to fill the gaps in communication between the facility, the patient, the family, and the clinician. “As a practicing physician assistant,  I appreciate that this service allows patients and families to address concerns or questions that may not be known or present during my regular scheduled facility presence. This has lead to a wholesome and proactive approach to patient care by giving everyone involved with the patient a voice and some answers." 

To qualify for CCM patients must be diagnosed with at least two chronic diseases, consent to the program, have a personalized care plan in a certified electronic health record, and have 24/7 access to a CCM care partner. The Premier CCM program is covered by Medicare and consists of a team of support clinicians that are available to patients and families that participate in the program all day, every day to hear their healthcare questions or concerns. In addition, the support team will call patients or their families monthly to review the patient's health status and report any relevant information to their Premier primary care clinician. 

The goal of the program is the early identification of opportunities to improve care resulting in costly transfers to the emergency department, hospital admissions, and to support a better quality of life for the patient.
