Premier Geriatric Solutions will expand its relationship with LTC ACO in 2023
For Press Release Roanoke, VA — December 20, 2022 - Doctor Bradley J. Goad, Premier’s Founder and Chief Executive Officer, announced that the partnership between Premier and LTC ACO, the first Accountable Care Organization (ACO) in the U.S. dedicated to long-term care facility residents, will expand dramatically in 2023. Dr. Goad noted that a medical practice Premier acquired effective July 1, 2022 (Valley Family & Elder Care), had an established relationship with LTC ACO that will experience dramatic growth as Medicare enrollees under Premier clinician’s professional medical care throughout Premier’s service areas in Virginia and North Carolina are added to the ACO’s enrollment.
In his remarks, Dr. Goad noted that the opportunity to receive actionable data and insights from LTC ACO, an organization managing more than 20,000 Medicare fee-for-service beneficiaries under the Medicare Shared Savings Plan (MSSP), will assist Premier to meet its goal of optimizing the health and cognitive status of every patient Premier clinicians serve.
“We’re thrilled to deepen our relationship with Premier and we greatly appreciate the trust they have put into LTC ACO to help them create value for patients,” said Jason Feuerman, President and CEO of LTC ACO. “We look forward to our continued work with Premier and helping the organization advance the state of healthcare for residents in its care.”
Premier is an independent post-acute/long-term care medical practice that is committed to optimizing the physical and mental wellbeing of the residents of eldercare communities including skilled nursing, long-term, assisted-living, and independent living. In 2022 they expanded to managing Premier patients’ transition home with their Transition of Care program. Premier is an innovative organization that has fully adopted a cloud-based electronic health record, implemented mental health and chronic care management services and is currently launching remote patient monitoring services. While technology and innovation are important, Premier also understands the importance of being a valued strategic partner to the staff and leadership of Premier partner eldercare communities.
LTC ACO began in 2016 as the first Medicare-approved Accountable Care Organization focused specifically on the special needs of Medicare beneficiaries residing in long-term care facilities. This includes, but is not limited to, the frail elderly, individuals living with Alzheimer’s disease or dementia, depression, amputations, developmental or physical disabilities, renal failure, and individuals with multiple chronic diseases with complications. By participating in LTC ACO, physicians and LTC facilities are recognized and rewarded for improving the quality and cost of care delivered to this vulnerable subset of the Medicare population.
